Choose and Book is a national electronic referral service that allows a patient the freedom to choose the place, date and time of their first outpatient hospital or clinic appointment, after a referral has been made by their GP.
If you have received a letter with a password and reference number from one of our Surgeries further to a referral from your Doctor, you can access the Choose and Book system and make your appointment by following this link
