Why do GPs sometimes charge fees? Although the National Health Service provides most healthcare to most people free of charge, there are some exceptions. In the case of the vaccines listed this is because these services are not covered by the NHS. Click on the link to view or download details of the services not covered and the fees charged. |

| Who do you need to see, a doctor or a nurse? | ||

It is not always necessary to wait for an appointment to see a doctor. In some instances it may be more appropriate and quicker for you to see one of our team of practice nurses. View or download our list showing who you would be best making your appointment with. |

To access our online services system you need to have a free online account with us. Download, print off and complete this form, then drop it into your usual surgery. Once the application is processed we'll be in touch. |

| New Patient Registration Form (University of York students only) | ||

| Repeat Prescription Form | ||
To order a repeat prescription, download this form and clearly indicate on it the items you require, then return it to your usual surgery. Please allow 2 working days for all prescriptions to be ready for collection. We are happy to post your prescription to you, if you require us to do this please enclose a stamped addressed envelope with your prescripton request. |
