Repeat Prescriptions

To order a repeat prescription, download the form below and clearly indicate on it the items you require then return it to your usual surgery.

Please allow TWO working days for all prescriptions to be ready for collection.  We are happy to post your prescription to you - if you require us to do this, please enclose a stamped addressed envelope with your prescription request.

You will be asked to attend for a review of your medication periodically at the discretion of the Doctor.

Some local pharmacies will collect and deliver your prescriptions; you should make these arrangements direct with the pharmacy in the first instance.
