New Registrations
All patients will be registered with the practice and not with an individual doctor. However, if you have a preference you can ask to be registered with a specific doctor. Although we will try to accommodate requests for appointments with specific doctors we cannot always guarantee this will be possible.

Repeat Prescriptions
Indicate clearly on the form provided the items you require and return it to your usual surgery. Please allow 2 working days for all prescriptions to be ready for collection. We are happy to post your prescription to you locally but please supply us with a stamped addressed envelope. You will be asked to attend for a review of your medication periodically at the discretion of the Doctor. Some local pharmacies will collect and deliver your prescriptions; you should make these arrangements direct with the pharmacy in the first instance.

Making an appointment
Call in and see the receptionist or phone the number on the back of this leaflet. We have an appointments system that allows patients to book an appointment on the day that they feel ill or need to be seen. This has allowed us to reduce the waiting time for appointments and means that we can treat acute health problems quickly without patients having to wait. However, this system does restrict the number of pre-book able appointments for those of you who need to book appointments around a busy lifestyle. Please try to book your appointment in advance if you require a specific time of day or if you would prefer to see a specific doctor. We can make appointments up to two weeks in advance.
IF YOU ARE UNABLE TO KEEP YOUR APPOINTMENT PLEASE LET US KNOW AS SOON AS POSSIBLE SO THAT WE CAN OFFER THAT APPOINTMENT TO SOMEONE ELSE.
The reception area can be very busy, we have improved confidentiality by providing music in the waiting room to mask conversations at the reception desk and at Wenlock Terrace and the University Health Centre we are able to provide a confidential enquiries desk away from the main desk that you may use if you would prefer.
PLEASE ADVISE THE RECEPTIONIST WHENEVER YOUR ADDRESS CHANGES SO THAT OUR CONTACT DETAILS ARE UP TO DATE.

Telephone Contact
Patients can contact doctors or nurses by telephone during normal opening hours by ringing their usual surgery when a convenient time can be arranged or their contact details can be passed onto the doctor or nurse to call back.
If you need to contact the surgery to enquire about the results of a test please wait until after 1pm to call as this is when we receive the information from the hospital laboratories.

Emergencies
During surgery opening hours please ring your usual surgery number. If you need a doctor very quickly please tell the receptionist who will inform the doctor as soon as possible.

Home Visits
If you feel too ill to come to the surgery a home visit may be arranged. Please telephone your usual surgery before 10:00 if at all possible leaving a contact number. The Doctor may wish to discuss your problem with you before arranging a visit.

When the surgery is closed
If you need a Doctor urgently you should telephone 0845 056 8060 which will connect you to the emergency doctors service, someone there will be happy to help you.
From 1.10.04 the out of hours service is provided by Selby & York Primary Care Trust, it is provided on the same number given above - 0845 056 8060. If you need to see a doctor between 6:30pm and 8:00am Monday evening to Friday morning or between 6:30pm Friday and 08:00 Monday then please call them on 0845 056 8060.

Are you a carer?
We have a Carer's notice board in each waiting room with useful information & telephone numbers. If you let the receptionist know that you are a carer we can log this information and it may help us to help you if you need assistance.
