To order a repeat prescription, download the form below and clearly indicate on it the items you require then return it to your usual surgery.
Please allow 2 working days for all prescriptions to be ready for collection. We are happy to post your prescription to you, if you require us to do this, please enclose a stamped addressed envelope with your prescription request.
You will be asked to attend for a review of your medication periodically at the discretion of the Doctor.
Some local pharmacies will collect and deliver your prescriptions; you should make these arrangements direct with the pharmacy in the first instance.
