If you can answer YES to the above 2 statements and wish to become a patient at the practice you need to download the registration form, complete and sign this then bring it into one of our surgeries to make an appointment.
We have a team of receptionists who will be happy to help you with any queries. Please help them if they ask you for more information - it is to assist the doctor in assessing your needs.
If you have difficulties with the service please contact our Reception Services Manager, Mrs Kirsty Holliday at the Wenlock Terrace Surgery or you can contact the Practice Manager, Mrs Brenda Mumby, at the University Health Centre.
We have a team of nurses who are trained to deal with many minor illnesses and can also offer contraceptive advice, asthma checks, blood pressure checks, diabetic checks, travel advice, routine immunisation and general health advice. Some of our nurses are able to prescribe many medications themselves, all of our nurses follow practice protocols that allow them to organise some prescriptions through your doctor.
